To put it short and simple: You start a transaction through the Trade Guardian System and then agree who of the two involved parties pays the fee. Once the fee is paid a MM has to claim the transaction. Once a Middleman has claimed the seller has to provide the Item/account to the Middleman who is then responsible in securing it. After that its the buyer turn to pay the seller directly. When all of this is completed the buyer will receive then their item from the Middleman.

When the Middleman secures the account (We check the account within the game, it can be any game) We protect it, that is, we change recovery options and thats it

How far does the middleman go when it comes to verifying the transaction, for example, I am trying to sell an Eve account - what is to stop a potential buyer from draining the account of resources, other assets, etc, and then claiming that the item was not as described and demanding a refund? What steps can I take to ensure this wouldn't happen?

The middlemen logs in to the account and sends the buyer screenshots of the contents of the account. The buyer has to verify then based on the screenshots or videos, if that is the real account, based on what the seller told them and showed, that they are purchasing. So claiming that what they received from the Middlemen isn’t as described won’t work.
