[Question] How do scammers do it?

Hi all, I'm new to the forums and to the trading business.

I went through the trade forums and came accross several posts about successful transactions but also several scams. I also read the stickies, but I have several unanswered questions.

How do scammers take over the account that has just been traded? I mean, say I am trading my acc with guy X, he gives me his username & pass and I give him mine. I change the password straight away. How can a scammer log in the account and change the password (since it has been changed)?

Also I came accross the term "reclaimed", what exactly is this?

Finally, has anybody managed to get his money back after a scammed trade over paypal?

Edit: Another thing which isn't very obvious from the stickies and the posts, how could a "middleman" potentially prevent a scam from occuring?

I think discussing these, common for some, practices will help many avoid being scammed and most definitely me who I'm a noob.
